• A non-refundable bank challan of Rs. 2500/- must be downloaded and deposited into HBL Account No. 1439-79013279-01 titled "PUMHSW Other Receipts Account".
• Send the printed application form along with the original paid bank challan, updated CV, three recent passport-size photographs, one set of attested photocopies of academic degrees/certificates, marks sheets, experience certificates, valid PM&DC registration, CNIC, and Domicile/PRC via certified mail/courier service to:
